AI Hallucination
An AI hallucination is an answer an AI states confidently and plausibly, but that is factually wrong or made up. The model then “guesses” an answer instead of admitting it lacks the information. In customer service that's risky, RAG, a confidence score, and human handoff keep it in check.
What is an AI hallucination?
We talk about an AI hallucination when an AI delivers an answer that sounds convincing but isn't true. The tricky part: the answer is linguistically flawless and comes across as assured. Only the content is wrong or invented. A customer often doesn't notice the error.
The reason lies in how a Large Language Model works. It predicts the most likely next word and is designed to always produce a fluent answer, even when it lacks the necessary information. Instead of saying “I don't know that,” it fills the gap with a plausible guess. That is exactly the hallucination.
Why is this critical for businesses?
In customer service, a made-up answer can do real damage: a wrongly quoted price, a promised delivery time that can't be met, or an invented returns deadline. That costs trust and, in the worst case, money. So you need safeguards. Three work particularly well:
- RAG. The AI answers from your own knowledge base instead of general knowledge. If it finds no matching source, it has nothing to invent from.
- Confidence score. Every answer gets a certainty value. If it's too low, the AI doesn't answer itself.
- Human handoff. If the AI is uncertain, it passes the conversation to a team member instead of guessing.
Clean prompt engineering also helps: a clear instruction like “If you're not sure, hand off to a human” noticeably reduces hallucinations.

A travel agency in Edinburgh uses an AI chatbot for initial enquiries. A customer asks: “Do I need a visa for Canada?” Without safeguards, the model might invent a specific but outdated answer.
